GENERAL REQUIREMENTS FOR VISA All visa requests must be submitted by mail only Documents and requirements 1 - Valid passport (with TWO blank visa pages) 2 - Printed confirmation page of online application form (see item Visa Form), clic: https://formulario-mre.serpro.gov.br/sci/pages/web/pacomPasesWebInicial.jsf 3 - One 2” x 2” passport photos 4 - Printed itinerary 5 - US Postal Service Money Order for payment of fee (if applicable) If applying with a passport not issued by the United States or Canada, also bring: 6 - Original and copy of valid US visa, permanent resident card, certificate of naturalization, passport or birth certificate 7 - Bank statements from the past 3 (three) months If applying for a minor (under 18), also bring: 8 - Original and copy of birth certificate (or other legal document proving guardianship) 9 - Signed and notarized consent form, as described below, cllic: https://sistemas.mre.gov.br/kitweb/datafiles/Hartford/en-us/file/Visa%20for%20minors.pdf If your visa will be submitted by a third party, you have to follow all the requirements above.
