Declaration of Accuracy form: fill it out, and sign.
Visa Fees: 80 EUR payable in money order. Most foreign missions only accept USPS money order.
Proof of employment: indicating that you're permitted to go on leave for the duration of your trip.
Proof of address: State ID, Driver's license, or a copy of your lease.

Notarized application form and US residence copy (Green Card or US visa).
